Obtain LingCare home care services through a government-subsidised Home Care Package, or by paying for yourself (self-funding).

Our service packages suit specific care needs including nursing or ancillary care, or domestic help, and our carers have familiar cultural and language abilities.

How to obtain our service

Everyone’s situation is different. The Australian government’s subsidy for your loved one’s services will depend on personal circumstances.

The Australian government manages the assessment through My Aged Care, and the level of home care package is determined by an Aged Care Assessment Team (ACAT).

Depending on the recipient’s circumstances, they may need to make a co-payment contribution, which will be assessed within government guidelines.

Getting started

Step 1: Call LingCare

  • LingCare affiliated medical professionals will assist your application to My Aged Care to work out your loved one’s needs and care arrangements. This makes your application simple and easy.
  • Our medical professionals will make direct online referrals to My Aged Care based on your loved one’s medical condition and care needs.
  • What to bring: Medicare card, relevant medical records e.g. referral letter, care plan, clinical notes.
  • Alternatively, you may contact My Aged Care directly on 1800 700 422.

Step 2: Have a face-to-face assessment

  • My Aged Care will arrange for an ACAT assessor to come to your loved one’s home.
  • With their consent the assessor will assess their care needs and eligibility for services, and work with you and your loved one to develop a support plan that addresses their needs, goals and preferences.
  • You can be with your loved one during this visit.

Step 3: Find out about
costs

  • My Aged Care and service providers can give you information about costs. You will be told if a financial assessment is needed.

Step 4: Choose LingCare as your service provider

  • When your loved one is assessed for a home care package, you should tell the My Aged Care team that you would like LingCare to be your service provider.
  • After home care package approval, the LingCare team will tailor the services to suit your needs.

How to change from your current service provider to LingCare

  1. Before you and your loved one decide to change providers, you should check your Home Care Agreement to find out if there are any conditions such as notice periods and exit amounts.
  2. To change from your current service provider to a new provider, you must contact My Aged Care on 1800 700 422 to re-activate your loved one’s referral code and mention that LingCare is the newly selected provider.
  3. Your current service provider must help you in the changeover of providers, e.g. regarding the date you want your current care and services to end.
  4. In changing over, your current provider must provide you with a statement showing the amount of unspent funds that will be transferred to your new provider.

Private/Self-funding – FlexiCare

Clients may choose to pay for the services themselves. With a self-funding home care program, the care service provided is not government-subsidised.

They may also include one or more extra services that are not covered by their government-subsidised home care package.

Obtaining these services and any fees that may be applied should be discussed directly with the home care provider.

Our Home Care Team will discuss your loved one’s care needs with you, and the services will be provided within the agreed package level to assist them to stay in their own home.

What are Home Care Packages (HCPs)?

Home Care Package (HCP) is an aged care service partially subsidised by the Australian Government. Obtaining these services may have specific eligibility requirements (such as age) and be subject to standardised fee arrangements.

Home care packages combine a range of support services tailored to meet specific needs to provide choice and control to support your loved one’s needs, and to assist them to stay independent in their own home. Services include:

• Personal Care – help with showering, dressing, mobility, meal preparation and eating.

• Support Services – help with washing and ironing, house cleaning, basic home maintenance, modifications (such as hand rails in the shower), transport for shopping, appointments or to attend social events.

• Clinical Care – nursing or other allied health support such as physiotherapy (exercise, mobility, strength and balance) and podiatry (foot care). See LingCare Services.

There are four levels of Home Care Package:

• Level 1 – basic care needs

• Level 2 – low-level care needs

•Level 3 – intermediate care needs

• Level 4 – high-level care needs.

The government subsidy for the specific home care package is paid direct to the selected approved home care provider. The subsidy contributes towards the cost of services and care delivery. It is expected that clients will contribute to the cost of the care service where their personal circumstances allow.

Home care packages are provided on a consumer directed care basis. That is, LingCare will work with you and your loved one to identify and provide a home care package to meet their needs so they can live a more active and independent life.

Contributions

In a Home Care Package, the government pays for most of the costs of the care services provided. The recipient only needs to contribute a small amount if financially able to do so. The payment to be contributed will depend on the type of services received. The government subsidy is paid directly to the provider, and it is different for each level of home care package. The amount the client is expected to pay is based on the maximum income-tested care fee determined by the Department of Human Services (see MyAgedCare fee estimator)
