LingCare After-Hospital Care

We provide continued care and support for your loved ones at home
to live independently after their hospital stay.

Services include transport and assistance for appointments and other activities, food shopping and meal preparation, general help at home, personal care, nursing and clinical needs, medical alert devices and home safety modifications.

After-hospital care helps minimise prolonged hospital stay, and allows continued support for your loved one in their own home to recover and improve their wellbeing. It can also allow sufficient time to organise eventual care arrangements.

How to obtain continuous after-hospital care

To obtain approval for our after-hospital care, you should make arrangements before your loved one is discharged from hospital. In discussion about their care plan with hospital personnel or social worker, you should request referral to My Aged Care regarding ACAT assessment and nominating LingCare as the home care provider.

Your loved one can obtain after-hospital care through a Home Care Package (HCP) to top-up, or by paying for themselves (self-funding). Our Home Care Advisor can help in the assessment if your loved one is entitled to government funding.
